What We Offer

Precise Care has accommodations for male/female, Level 3, 4i, and specialized medical, ambulatory and/or non-ambulatory residents between the ages of 18 and 59. Residents must have been diagnosed as developmentally disabled as defined in Section 4512 of the California Welfare and Institutions Code and in need of incidental medical care.

The following needs and services include:

  • Resources for academic enhancement

  • Vocational skills

  • Resident counsel meetings

  • Interpersonal relationships

  • Behavioral programming

  • Individualized positive behavior support plans

  • Monthly incentive programs/awards, and recognition

  • Residential birthday celebrations

Service Philosophy

Precise Care is an adult residential facility whose sole purpose is to refine and reinforce living conditions for intellectually disabled and developmentally disabled adults. Our philosophy is to allow our residents to explore and express their human rights, while emphasizing respect for their peers.  

In implementing this philosophy, we employ the following methods: 

  • Provide individualized positive behavioral support plans

  • Home environment that fosters individuality, compassion and inclusivity

  • Trained professionals to provide personalized care and support

  • Ongoing support for self advocacy

  • Promote coordination of care among all healthcare professionals, educators and support personnel

  • Encourage an open, ongoing line of communication with caregivers, guardians, and/or responsible persons

  • Maintain accurate and efficient documentation

  • Employ an interdisciplinary approach to the development of resident and staff, and coordinate programs with outside resources

 
 

Anticipated Goals & Outcomes

The anticipated outcome of all programs incorporated at Precise Care is to support and enhance the resident's quality of life. Through intervention, redirection, and positive reinforcements, all programming methods used by Precise Care will be directed toward modifying and replacing inappropriate behavioral excess. In an effort to cultivate a more confident decision-making individual, residents are guided to increase their abilities in a number of ways; thereby decreasing their need for a restrictive environment. 

Residents are supported in: 

  • Reaching their Individual Program Plan (IPP)

  • Decreasing inappropriate behavior

  • Increasing self-esteem

  • Acknowledging and understanding their mental state and current medical condition

  • Establishing and strengthening independent living skills
